The Supported Individual’s ambition or effort; an aim or desired result is how the CLS defines a goal. All Supported Individuals have at least one goal outlined that they will be working on. Each goal will be measurable and tailored to the Supported Individual’s aspirations and needs, focusing on enhancing socio-emotional well-being and personal development.
Annually, at a minimum, the Supported Individual and their personal support network (if applicable) review goals as part of the ISQLP meeting.
Each goal plan includes five components:
- Why the goal is important and meaningful to the Supported Individual, the purpose.
- What goal will be achieved in one year, what the goal is.
- How the Supported Individual will work towards achieving the goal, outlining any steps required.
- Who is responsible for completing the steps, sometimes this is the Supported Individual and sometimes the employee.
- How the Progress will be tracked on activity logs and by the Leaders.
